SMFA students interested in joining sports teams at Tufts should contact the coach of the team. Click here for information about the different sports offered and contact information for all coaches.

To join sports teams at Tufts, specific health documentation is required. The Tufts Health Center cannot see SMFA students. All medical records must be gathered from the student’s own doctor. To get specific instructions on the records required, contact Mary Daly at the Tufts Health Center (phone 617-627-3350 or fax 617-627-3592).

At a minimum, students are required to provide a copy of their last physical, immunization records, and a completed sports questionnaire. The Tufts sports questionnaire is available online at http://ase.tufts.edu/healthservices. In the right-hand column click on "forms," then follow the link at the bottom of the page to "misc forms" and select "sports questionnaire."
